SAC is excited to announce that we are scheduling outdoor visits!  We are following the State of Indiana requirements to ensure your safety, as well as the safety of our residents and staff.

If you have Covid-19 symptoms we ask that you do not visit.

Residents who are completing a 14-day quarantine (post hospital stay, new admission, etc…) will be able to participate in the outdoor visits once the quarantine has been completed.

Visits must be scheduled one week in advance.  We ask that visitors limit their visit to once within a 2 week/14 day period. A maximum of 2 guests is approved during the scheduled visit. Visitors must be 18 years of age or older unless approved by our Administrator.

Masks must be worn during the duration of the visit.  Please bring your own mask.  We will not provide you with a mask.

Check-in at door 13 (Rehabilitation Suites) when you arrive for your scheduled visit.  Your temperature will be taken and you will be required to answer a series of questions regarding your health and potential covid-19 exposure.

Once you have completed the check-in process you will be instructed to go to Door 1 or Door 7 for your visit.

Chairs and a table will be provided.  These will be cleaned after each visit.

Kleenex and hand sanitizer will be available.  Hand sanitizer should be used prior to and after your visit.  If you sneeze, please cover your mask with a tissue.

You must practice social distancing during your visit and remain 6 feet from your loved one.

If you need to cancel your scheduled visit, please contact 484-5555 and follow the prompts to speak to our receptionist, or cancel your reservation on-line.
