  • Graduation from a State accredited school of Professional Nursing with a current, valid license to practice as a Licensed Professional Nurse (LPN) in the State of Indiana
  • Completed an approved medication course
  • CPR – Healthcare Provider & First Aid Certification
  • 3(+) years experience in a long-term care facility (preferred)



  • Administer and document medications given
  • Stay awake and alert throughout shift
  • Operate special equipment for procedures, suction machine, feeding pumps, IV pumps, ambu-bag, emergency drug box, air beds, internal feedings and irrigations, insertion, installation, and irrigation of urinary catheters, glucoscan tests, etc.
  • Administer special skin care procedures
  • Perform sterile dressing changes
  • Conduct assessments, monitor residents for change in condition, and contribute to ongoing summary documentation
  • Co-sign QMA summaries
  • Plan and provide care which meets physical, psychological, spiritual and rehabilitative needs
  • Communicate effectively with residents, physicians, families, and co-workers both verbally, and in writing
  • Collaborate with the team and take direction from the Charge Nurse and/or DON
  • Represent and promote the facility in a positive manner
