GENERAL STATEMENT OF PHYSICAL REQUIREMENTS:
This position requires assisting in lifting residents in areas of 25 pounds frequently and occasionally in excess of 100 pounds with frequent lifting and/or carrying of objects weighing 10 pounds or more. It requires walking or standing as an essential function with a significant degree of pushing and pulling of arms and/or legs. In a work day, an employee must stand/walk 6-8 hours; sit 1-3 hours; use hands for repetitive grasping, pushing and pulling; employee must also be able to bend frequently, kneel occasionally, twist frequently and reach/lift frequently. Wearing protective clothing such as a gown, mask, eye shield, and glasses is an essential function of this position. Frequent washing of hands is essential.

ESSENTIAL FUNCTIONS:
- Provides personal care for the residents:
- Provides daily hygiene measures including bathing, oral hygiene, skin care, hair and nail care as assigned and as required.
- Changes bed linens as required.
- Dresses/undresses residents as needed.
- Assists in feeding of residents or with additional food/fluids.
- Answers residents’ lights promptly.
- Provides morning and evening care measures as appropriate.
- Assists co-workers when necessary to complete assignments.
- Maintains Resident unit and Nursing work areas in a clean, orderly and safe manner:
- Uses only necessary items for care.
- Replaces furniture to proper safe locations.
- Handles equipment and supplies to avoid waste or breakage.
- Uses time effectively and efficiently.
- Assists Residents with preventative, therapeutic and restorative procedures:
- Takes vital signs, weights on resident admissions and at required intervals.
- Performs resident treatments not requiring medication.
- Uses procedure manual to verify correct techniques.
- Keeps residents’ skin clean and dry.
- Performs range of joint motion exercises daily as needed.
- Assists residents to ambulate or use assistive devices.
- Assists the nurse in bowel/bladder rehabilitation programs.
- Adheres to safety and infection control policies and procedures:
- Maintains good personal hygiene and adheres to uniform requirement.
- Practices medical asepsis.
- Changes resident’s ice water each shift.
- Performs routine cleaning of resident bedside unit, care equipment and utility rooms.
- Follows infection control policies and procedures.
- Reports personal illness or skin lesions.
- Attends in-service programs as required.
- Reports defective equipment or other potential safety hazards promptly.
- Follows universal precautions.
- Washes hands immediately after removal of gloves.
- Communicates effectively orally and in writing:
- Reports changes in resident condition or unusual signs, symptoms, accidents or incidents.
- Documents resident nutrition and fluid intake and reports as necessary.
- Documents daily on Facility forms and resident medical record.
- Clarifies questions regarding assignment with Charge Nurse.
- Maintains confidentiality of information related to residents.
- Promotes positive public relations for the Facility:
- Gives residents choices.
- Displays courteous, tactful, helpful manner with residents and visitors.
- Uses proper telephone etiquette.
- Represents the Facility in a positive manner.
- Maintains confidentiality of all matters pertaining to resident condition and personal affairs.
- Maintains neat, clean and professional attire, including appropriate uniform.
- Performs other tasks as required:
- Assists other personnel when assignment completed.
- Participates in one-to-one activities with residents.
- Assists in transferring residents if help in lifting is needed.
- Assists where resident has fallen to the floor.
- Responds to the fire alarm.
JOB QUALIFICATIONS:
- EDUCATION AND FORMAL TRAINING: Completion of State approved Certified Nursing Assistant course as evidenced by current State certification as a Nursing Assistant – 105-hour course.
- EXPERIENCE: At least two years’ experience as a C.N.A. in a long-term care Facility is preferred.
- OTHER:
- Ability to communicate effectively and work as a team member.
- Ability to take direction from the Charge Nurse and also take initiative to work independently and responsibly.

GENERAL STATEMENT OF DUTIES:
- Administers and documents medications given
- Attends report.
- Performs Class II medication count at beginning and end of each shift.
- Follows all pharmacy and drug administration policies and procedures.
- Specifically follows physician orders.
- Administers oral, topical, suppository medications as well as eye and ear drops.
- Adheres to specific administration requirements, including before meal(s), as well as correct time of administration.
- Confers with licensed nurse before administering PRN medications and PRN treatments, including any assessment, and have nurse co-sign administration sheet.
- Documents all medications administered according to Facility policies and procedures.
- Uses resource materials to verify drug information as well confer with Nurse for questions.
- Performs treatments as assigned:
- Prepares and administers topical medications.
- Uses procedure manual to verify correct techniques.
- Performs medicated shampoo and bath treatments.
- Observes, records and reports observations:
- Monitors residents for expected and outward effects from medications.
- Takes vital signs as needed before administration of certain medications.
- Checks with Nurse before administering medications when there is a concern or question.
- Observes, records and reports to Nurse any change in condition and/or pertinent information. Obtain Nurse signature and verify information documented.
- Provides direct resident care:
- Functions as a nursing assistant when not assigned to pass medication – See CNA duties.
- Assists other nursing assistants with work area needs when not involved in passing medications.
- Performs resident care measures as directed by the Nurse.
- Adheres to safety and infection control policies and procedures:
- Practices good personal hygiene and adheres to uniform requirements.
- Attends regular and medication in-services as required.
- Follows Facility infection control policies and procedures.
- Reports personal illness or skin lesions.
- Reports defective equipment or other potential safety hazards promptly.
- Promotes positive public relations for the Facility:
- Gives residents choices.
- Displays courteous, tactful, helpful manner with residents and visitors.
- Uses proper telephone etiquette.
- Maintains confidentiality of all matter pertaining to resident condition and personal affairs.

TASKS A QUALIFIED MEDICATION AIDE MAY NOT PERFORM:
- Call physicians.

- Cannot evaluate or treat decubitus Stage II and above. Stage I can apply lotions but evaluation done by nurse. Nurse must co-sign documentation on resident condition evaluation.

JOB QUALIFICATIONS:
- EDUCATION AND FORMAL TRAINING: Completion of State approved course in medication
administration as evidenced by current State Certification as a Qualified Medication Aide.
- EXPERIENCE: One to two years experience as a Qualified Medication Aide in a long-term facility is preferred.
- OTHER:
- Ability to effectively communicate with residents and co-workers both verbally and in writing.
- Ability to work as a team member.

Essential Functions:

Employee must be able to communicate on the telephone effectively and must be able to operate and/or work with nursing machinery and equipment as well as wear protective clothing such as gown, mask, eye shield and gloves.

Primary Job Duties:
- Administers and documents medications given.
- Executes physician orders thoroughly and safely.
- Administers and documents all types of medications according to pharmacy and drug administration policies and procedures.
- Contacts physicians and/or pharmacies as necessary for orders or resident prescription needs.
- Receives telephone orders from physician.
- Uses drug resource materials to ensure knowledge regarding action, usual dosage, and side effects of drugs.
- Ensures safe storage of medications.
- Attends regular and special medication in-services as required.
- Performs treatments, assists physicians and provides hands on resident care as needed.
- Utilizes all access points in Point-Click-Care to chart resident progress, not limited to, but including medications, behaviors, orders, documents, etc.
- Performs all treatments that may not be done by Certified Nursing Assistants:
- Glucoscan test
- Internal feedings and irrigations
- Insertion, installation and irrigation of urinary catheters
- Administers special skin care procedures
- Performs sterile dressing changes
- Monitors, maintains and d/c IV fluid administration
- Oral-pharyngeal suctioning
- Tracheotomy care
- Skin protective devices
- Administers medications/fluids through Heparin Loc
- Monitors central lines
- Administers oxygen
- Medicated shampoo and bath treatments
- Operates special equipment for procedures, suction machine, feeding pumps, IV pumps, ambu bag, emergency drug box, air beds, etc.
- Follows Facility policy and procedures.
- Assists physicians with examinations and procedures.
- Promotes cost containment through careful and efficient use of supplies and time.
- Observes, records and reports observations.
- Monitors resident for expected and outward responses from medications and therapeutic measures.
- Conducts assessments, notifies appropriate parties an documents when change in condition is noted.
- Notifies Charge Nurse of any change in resident condition with any additional pertinent information.
- Gives oral reports of resident condition to Charge Nurse and Certified Nursing Assistants as required.
- Contributes to completion of Facility forms, such as accident/incident, decubitis report or infection control, B/B, skin, fall risk, restraints, etc.
- Contributes to ongoing summary documentation nursing notes as required for MDS/PPS purposes.
- Co-signs QMA summaries.
- Assesses resident needs; plans and provides resident care.
- Assists with admission assessments and formulation of the plan of care.
- Contributes information for revision in the resident plan of care.
- Coordinates activities with Certified Nursing Assistants to provide comprehensive, effective and efficient resident care.
- Plans and provides care which meets physical, psychological, spiritual and rehabilitative needs.
- Uses opportunities for resident education in therapeutic and preventative health measures in collaboration with the Charge Nurse.
- Adheres to safety and infection control policies.
- Practices good personal hygiene and adheres to uniform requirements.
- Attends regular and medication in-services as required.
- Follows Facility infection control policies and procedures.
- Reports personal illness or skin lesions.
- Reports defective equipment or other potential safety hazards promptly.
- Initiates measures to protect residents from potential self-harm or harm to others.
- Promotes positive public relations for the Facility.
- Gives residents choices.
- Promotes options to residents so as to convey possibilities versus limitations.
- Retains familiarity with Facility, organizational structure, Directors and resource persons to ensure optimum communication within Facility and in dealing with the public.
- Maintains attitude of courtesy, tact and helpfulness when dealing with residents, visitors, physicians, therapists, consultants and other Facility staff.
- Performs other tasks as required.
- Transcribes physician orders.
JOB QUALIFICATIONS:
EDUCATION AND FORMAL TRAINING: Graduation from a State accredited school with a current, valid license to practice as a licensed professional nurse (LPN) or a registered nurse (RN) in the State of Indiana. Must have completed an approved medication course. CPR Healthcare Provider and First Aid Certification.
EXPERIENCE: minimum of three years in a long-term care facility, preferred
OTHER: Possess the ability to communicate effectively with residents, physicians, families and co-workers both verbally and in writing. Possess the ability to work as a team member and to take direction from the Charge Nurse, ADON, and/or DON.

GENERAL STATEMENT OF PHYSICAL REQUIREMENTS:
This Position requires lifting 80 pounds occasionally with occasional lifting/carrying objects up to 70 pounds. This job also requires standing /walking up to 8 hours, sitting up to 3 hours, and driving occasionally 1 hour daily. This employee must use hands for grasping, pushing, pulling, and fine manipulation. Employee sometimes uses feet to operate equipment and vehicle controls. Employee should be able to frequently bend, kneel, climb, twist, reach, and lift approved weight above shoulder level. Employee will be exposed to hot and cold conditions inside and outside of facilities.

PRIMARY DUTIES:
- Helps department fulfill its main obligation of supporting the operation of the Facility.
- If assigned, will primarily perform painting and painting maintenance as directed by Supervisor.
- At all times, performs work safely with high quality and speed maintaining a safe/clean environment.
- Communicates problems and provides one or more workable solutions to Supervisor.
- Accurately estimates time needed for completion on an assignment or task.
- Determines the parts required for assigned task in advance.

- Assists in grounds maintenance – lawn care and snow and ice removal operations as required.

- Operates mowers, tractor and snow plow as required.
- Installs, repairs, and rebuilds pumps, fans and similar mechanical equipment.
- Performs routine maintenance and repair on the Facility and equipment including plumbing, painting, wall papering, plastering, electrical, carpentry, and mechanical.
- Follows established safety procedures and precautions when performing maintenance services.
- Maintains Facility in good repair ensuring a safe, clean and orderly environment.
- Reports all incidents or accidents no matter how minor to the Supervisor.
- Services the heating and cooling systems.
- Performs unscheduled maintenance tasks as directed.
- Assures that the Facility and its equipment are properly maintained for resident comfort and convenience.
- Assures that the Facility and its equipment, tools and supplies are properly stored before leaving such areas for breaks or at the end of the workday.
- Follows established policies and procedures concerning infection control when performing maintenance services in isolation rooms or areas.
- Performs other related duties as assigned or directed by Supervisor or Administrator.
OCCASIONAL FUNCTIONS:
- Attends at least one (1) seminar a year.
- Assists when needed in training others.
- Draws from past information for analysis of chronic problems.
- Schedules work to complete assignments and/or activities.
- Replaces burn out light bulbs throughout Facility.
- Attends all in-services.
JOB QUALIFICATIONS:
- EDUCATION AND FORMAL TRAINING: High School graduate preferred.
- EXPERIENCE:
- Two years experience in equipment repair.
- Basic knowledge and experience in electrical, plumbing, carpentry, heating and cooling.
- OTHER:
- Ability to read blue prints
- Ability to bend conduit.
- Ability to communicate effectively with co-workers and management.
- Ability to use a computer.
- Ability to work as a team with co-workers.
