Optimus: Access to areas as assigned to MDS Nurses
ESSENTIAL FUNCTIONS:
- Will work within the scope of practice of the Nurse job description.
- Overall management/oversight of entire MDS process for designated area. This should be done using appropriate organizational systems, including but not limited to calendars, spreadsheets, monthly/weekly/daily lists, tickler files, etc. As a general rule, this position will not be completing MDS documentation on a regular basis.

Position Summary: This position will help lead the coordination of ongoing and outstanding customer experience in the Dining Services Department (LTC) at Saint Anne Communities. In collaboration with the Registered Dietitian and other dining supervisors, this position will identify, collect, analyze, and report key dining metrics to ensure the highest dining experience in compliance with all applicable state and federal health regulations.

POSITION SUMMARY:
The Pastoral Care & Admissions Coordinator for the End-of-Life Suites is responsible for overseeing all non-clinical operations for a 5-bed licensed SNF/NF unit dedicated to residents receiving end-of-life care and hospice respite services within a Catholic healthcare ministry.
This role integrates compassionate admissions coordination with spiritual and pastoral support, ensuring that residents and families experience dignity, peace, and the healing presence of Christ during the final stage of life.
The Coordinator serves as the primary liaison for residents, families, hospice partners, clergy, and referral sources, while upholding the mission and values.
MISSION INTEGRATION
- Uphold and promote the Catholic identity and mission of the organization.
- Ensure care and decision-making reflect Catholic moral teaching and respect for the sanctity of life from conception to natural death.
- Facilitate access to the Sacraments and spiritual support for residents and families.

Essential Responsibilities
Admissions & Referral Coordination
- Serve as primary contact for all referrals to the St. Joseph Hospice Suites.
- Coordinate and complete the full admission process including:
- Inquiry management
- Insurance/benefit verification (Medicare, Medicaid, hospice, private pay)
- Pre-admission screenings
- Required documentation and consent forms
- Conduct compassionate tours and family consultations.
- Collaborate with hospice agencies to coordinate seamless transitions.
- Maintain census goals for the 5-bed unit.
- Develop referral relationships with hospitals, parishes, hospice providers, physicians, and community partners.
Pastoral Care & Spiritual Support
- Conduct spiritual assessments upon admission.
- Provide pastoral presence, prayer, and emotional support to residents and families.
- Coordinate visits from priests, deacons, Eucharistic ministers, and chaplains.
- Facilitate reception of the Sacraments (Anointing of the Sick, Eucharist, Reconciliation) when desired.
- Offer grief support and bereavement follow-up after a resident’s passing.
- Coordinate memorial practices in alignment with Catholic tradition.
Family Advocacy & Non-Clinical Oversight

- Collaborate with nursing leadership to ensure smooth communication.
- Participate in QAPI initiatives related to end-of-life services.
- Ensure compliance with SNF/NF regulations related to admissions and documentation.
Qualifications
Education:
- Bachelor’s degree in Theology, Pastoral Ministry, Social Work, Healthcare Administration, or related field preferred.
- Certification in pastoral care or chaplaincy preferred.
- Eucharistic Minister required
Experience:
- Experience in long-term care, hospice, pastoral ministry, or healthcare admissions preferred.
- Understanding of Catholic sacramental life and end-of-life ethical directives required.
- Knowledge of Medicare, Medicaid, and hospice reimbursement preferred.

The Director of Mission Integration, Pastoral Care & Development serves as the spiritual and relational leader of the organization, ensuring that all aspects of care reflect the dignity of the human person in accordance with Catholic teaching.
This role oversees the pastoral care program, including supervision of Pastoral Associates, while also leading outreach, community engagement, and development efforts that sustain and grow the ministry. Through compassionate presence, leadership, and storytelling, this position bridges bedside ministry with mission advancement.
Key Responsibilities
- Pastoral Care Leadership & Oversight
- Provide direct spiritual care to residents, families, and staff, especially at end of life
- Supervise and support Pastoral Associates, including:
- Scheduling coverage to ensure consistent pastoral presence
- Providing mentorship, formation, and guidance
- Ensuring quality, consistency, and documentation of spiritual care
- Coordinate access to the Sacraments (Mass, Confession, Anointing of the Sick)
- Oversee prayer opportunities (Adoration, rosary, devotions) across the community
- Ensure each resident’s spiritual needs are assessed and honored
- Lead regular pastoral care team meetings and case reviews

- Ensure all communications reflect the dignity and sacredness of the ministry
- Partner on newsletters, donor communications, and marketing efforts

- Admissions & Family Support Collaboration
- Partner with admissions to support families during transitions and decision-making
- Ensure a pastoral presence is available early in the admission process
- Help communicate the mission and value of Catholic end-of-life care
- Guide pastoral associates in supporting families consistently
Supervisory Responsibilities
- Directly supervises Pastoral Associates
- Responsible for:
- Hiring, onboarding, and training pastoral staff

Qualifications
- Practicing Catholic in good standing, committed to the teachings of the Church
- Background in theology, pastoral ministry, or related field preferred

OCCASIONAL FUNCTIONS:
- Responsible for entire department during scheduled shift on weekends or when Supervisor or Director of Dining Services are not present.

GENERAL STATEMENT OF PHYSICAL REQUIREMENTS:
This Position requires lifting 80 pounds occasionally with occasional lifting/carrying objects up to 70 pounds. This job also requires standing /walking up to 8 hours, sitting up to 3 hours, and driving occasionally 1 hour daily. - Maintains Facility in good repair ensuring a safe, clean and orderly environment.
- Reports all incidents or accidents no matter how minor to the Supervisor.
- Services the heating and cooling systems.
- Performs unscheduled maintenance tasks as directed.
- Assures that the Facility and its equipment are properly maintained for resident comfort and convenience.
- Assures that the Facility and its equipment, tools and supplies are properly stored before leaving such areas for breaks or at the end of the workday.
- Follows established policies and procedures concerning infection control when performing maintenance services in isolation rooms or areas.
- Performs other related duties as assigned or directed by Supervisor or Administrator.
OCCASIONAL FUNCTIONS:
- Attends at least one (1) seminar a year.
- Assists when needed in training others.
- Draws from past information for analysis of chronic problems.
- Schedules work to complete assignments and/or activities.
- Replaces burn out light bulbs throughout Facility.
- Attends all in-services.
JOB QUALIFICATIONS:

- Two years experience in equipment repair.
- Basic knowledge and experience in electrical, plumbing, carpentry, heating and cooling.
