24 Hour Nursing Care

Saint Anne Communities offers 24-hour nursing care at our Fort Wayne location. For over 50 years, our mission-minded staff has been providing quality support to those requiring around-the-clock care and supervision. Our Social Services and Pastoral Care teams are available to aid residents and families as more care is needed.

24 Hour Nursing Care

Residents enjoy activities tailored to their needs within a secured environment, where we provide comprehensive 24-hour care. This level of care may be needed for individuals who require more comprehensive medical care and monitoring than can be provided in the Assisted Living setting. We are licensed by the Indiana State Department of Health (ISDH) and are subject to an annual inspection to ensure we are in strict compliance with state and federal regulations.

Programming includes familiar activities centered on each resident’s normal day-to-day routines. Activities typically occur in small groups and may include pet therapy, music therapy, religious programs, gentle chair exercises, manicures, supervised cooking, reminiscence groups, and familiar games. Residents also have access to common areas, outdoor patios, and courtyards while accompanied by staff or family.

We provide a full range of services in a secure, helpful, and attentive environment designed exclusively for those struggling with the effects of dementia and/or Alzheimer’s disease.

At Randallia Place, we provide a full range of services in a secure, helpful and attentive environment. With respect for each individual’s need for structured routine and memory-prompting activities, we have designed a wide variety of programs that enable residents to live fulfilling and productive lives, maintain a level of independence, and preserve dignity.

The Courtyard apartments at Randallia Place are designed exclusively for those struggling with the effects of mild to moderate dementia and/or Alzheimer’s disease, yet are still able to complete some activities of daily living on their own, with cueing, or perhaps a little assistance. This small, cozy area of specialized care encompasses 14 separate apartments surrounding a lovely family room, home-like kitchen, and dining room.

Cherished Memories is an entire floor dedicated to those whose dementia has progressed to the point of needing more assistance and hands-on care. This memory support unit features a spacious activity room, café-like dining room, and specially trained staff to ensure that residents in each of the semi-private rooms receive quality care and are truly cherished as individuals.

Person-Centered Care

The person-centered care approach is a philosophy that focuses on the individual as a whole rather than focusing only on one’s illness or inability. Person-centered care is about doing things with a person rather than to them. This philosophy respects one’s preferences, values, family situations, and social circumstances, focusing on a team approach to care. The resident, their family, and our memory care staff work together to develop an appropriate plan of care.

Memory Care Services

Saint Anne Communities offers a full spectrum of rehabilitation services including physical, occupational and speech therapy. Our beautiful, state-of-the-art therapy center is over 3,500 sq. feet and well equipped with modern fitness technology used by elite athletes and trainers.

Short-term Rehabilitation is available for guests requiring additional therapy and healing time after a surgery or illness. Specializing in post-acute transitional rehab, Randallia Place offers 16 private, rehab suites with 24-hour nursing care and therapy up to 7 days a week. Our team consisting of therapists, nurses, physicians, and social service providers work closely together to meet your individual needs and recovery goals.

Comprehensive Therapy Services

PHYSICAL THERAPY

Physical therapists strengthen and target movements that can no longer be performed normally. They also prescribe exercise so that individuals can become independent and self-reliant.

OCCUPATIONAL THERAPY

The goal of Occupational Therapy is to provide individuals with the tools and techniques to return to normal function of everyday life. This could be: learning medication management after a surgery, or relearning how to dress, bathe, prepare meals, and perform light housekeeping duties.

SPEECH THERAPY

Speech therapy seeks to restore function for individuals that have difficulty communicating, swallowing, or eating. Speech therapy can help those recovering from a stroke, brain injury, or individuals who have difficulty communicating due to dementia. It can also help patients learn safe swallowing techniques to reduce choking risks.

OUTPATIENT THERAPY

Our therapy center is well-appointed to provide, comprehensive outpatient therapy services for our residents and the greater community. Assisted Living means that person may require some help from our nursing staff. Residents receive all of the amenities included in Independent Living and more without needing to move to another area. The resident would simply pay an additional fee for any of the following services:

  • Medication administration and monitoring
  • Assistance with bathing and/or dressing
  • Verbal cuing for those with mild cognitive impairment
  • Wheelchair transportation or stand-by assistance for ambulation
